Wow, I have never come real close to $600 in a year and I don't think  I will try after reading how complicated it is.

I did not do the Fax Verification.  I was able to email in.  Good idea  doing the support ticket.  I did the same thing when I reached $600.00.  Wanted to get what ever was needed because you cannot cashout until it is done
Do you have a email address for FusionCash?

If I remember correctly the instructions popped up when I tried to cash out.  Have you tried to cash out yet?

It's easy they send you all the paperwork in the mail after you sign the release online to do so. Not a hard thing to do

Most people don't have to worry about it, but if you are close to it at the end of the year, don't do your cash out until the new year then you stay under the $600 limit  8)

Gives me an error message stating I need to do a fax verification.  I think I have it straightened out.  I found the fax verification info.

Not here but I had to fill out a W4 form and received a 1099 misc from a survey panel I was a member of for making over $600.

I don't have to worry about that.  I just make over $100 a year on FC.  It gives me a little spending money. :)

It is NOT complicated. However, it will adversely affect your taxes, because you will have to pay taxes on that as income. The year that I claimed I think I had to pay about a third of that $600 in taxes... I wasn't able to roll it over to the next year without losing credits. Although after realizing how much it cost me, that would have been a cheaper way to go.

Ouch, a third? Yea.... then that doesn't necessarily make sense to want to cash out more than $600...
