i would steal a sugar packet and not even leave a tip before walking out too :P

We did have some water. lol.  And NO she didn't get a tip. What would she get it for?  I am not sure if she didn't get it turned in or if the kitchen lost it.  Either way I wasn't going to sit there any longer.  We were already pressed for time.

I will do 1 better........my wife and i once walked out of a pizza place waiting almost a half hr and our order wasnt even taken!!

So they probably didn't even know you were there.  I figure eventually she brought our food out and we were gone.  I can see her looking around wondering where we went to. lol  OR coming over again to tell us it would be out in a few more minutes.

It was not so much as just waiting for food as realizing after 30 minutes of waiting that our food had not even been started to be prepared.  That is what really got us.  She should have come over and told us what happened and that our meal would be free.  It was funny because while we were waiting I was reading reviews online and 90% of them were bad and for really really slow service.  And the place is not that busy.

I don't believe I've ever done so in a restaurant but I do recall walking out of a Dr's office
because of a similar issue.

My husband has threatened to walk out in the past.
If the waitress comes to us and gives an explanation as to why the delay (and it is reasonable), I can be more patient.
But if there is no customer service on the waitress's side or from management, then it is reasonable for patrons to leave.

I have only once but not before I asked for my money back. This one there was no other customers ordering in the evening the guys were just talking in back and the guy came up and said, they are working on huge order but just sitting and talking, it was almost 20 minutes nothing so I asked them that I am no longer impress by their service and needed my money back.

I would have asked for an update, after a reasonable amount of time.

A good food-server would have updated you on expected wait.

Accidents do happen, they can lose your order or something. However, the waiter should keep an eye out, and the customer needs to communicate.

I don't remember ever walking out of a restaurant, but I can understand your frustration especially if you were on a time schedule.  The place obviously needs to have some compulsory classes on customer service.  Keeping people waiting with no explanation is not acceptable.
