TV and Movie survey

Hi All,
 I am fairly new here, this is my first post so please bear with me. I just tried to take the FC survey called TV and Movie 15 mins for $1. I was asked for all my personals,email, which was ok then name & address, which really is not. Then the screen rolled to a survey for 30 minutes and less money. So my question is has any one seen this type of thing? Is it a fluke? Something for concern? I backed out of it, 'cuz plain and simple I'm not always so trusting! This survey is different than any other one that shows up on that page. I'm not sure how it works if it tells you a different amount to get paid. I submitted my information and it just keeps telling me I don't qualify, so I can't tell which amount you would get paid. You may get $1 through Fusion Cash and the other amount through the company administering the survey, but probably not. Just for future reference though, the surveys in that block are usually not like this. Usually, you click on the survey, it takes you right to the survey, and you complete the survey. Most of the time you don't have to sign up with an email and stuff. This one is an unusual circumstance, so don't let it discourage you.

I just took this and it wasn't "TV & MOVIE" subject based, it was about Kelly Blue Book and buying new cars. It's often confusing with FC lists a survey as a certain subject but it ends up being for something else. I'm sure this is a demographic purity tactic.

I clicked out of this one as soon as they asked for my address. I think asking for a zip would have been adequate, no need for address.
