I finally got my first referral. My $2 offer completion bonus is showing up, but not my $1 bonus for them confirming their email.
Where's my bonus?

They haven't filled in their profile number, confirmed the e-mail and/or phone number - Basically just everything stated above altogether.

Absolutely.  I have already been paid on four separate occasions for this.  As long as you make 30 posts in a given calendar month that are of the contributory nature, and fill out the request form once getting to those 30 posts, you should receive your $3.00 bonus.  Just remember, it can take up to 60 days to show in your account.
