I have tried countless times to cashout. I have sent many support tickets. I even printed out a tax form, filled it out and offered to mail it to them. I could have walked their by now. I asked for a physical address so I can mail the tax form in. this is what they sent me.

when I click it. it comes up invalid. I don't see what the problem is. why cant I just mail in the form?. I have over a hundred dollars and don't want to lose it.

I believe that you have posted about this issue previously in forum. I am sorry that it still has not been resolved. One of the members, ladavia89, posted a link to the form that you need to fill out. I tried the link and it brought me to the correct form I have no reason to fill it out but it looked simple enough and there is a submit button at the bottom.  Have you tried this link?

I tried this link and I get the message about the fax verification method. I asked fc what this was and they just keep giving me the same link. I am like a dog chasing my own tail

I would try the fax number and if that
does not work, try to send a regular
mail.

I don't know if I'm getting what the issue is, can't you just fax the info to them and you will receive your cashout? A lot of people have posted about the fax verification and needing to complete it.
