Had cashout pending, but it never came through. Please help

I requested a cashout (check) at the end of April. (I had more than $25 to cashout and more than $15 were in actual surveys.)
My account said pending cashout in April. It stayed pending for a long while. Then May 20 came and went. Here we are June 4 and my account no longer says pending, it says "approved" next to each of my items and says that I have cashed out $0.00. I have not received a check in the mail yet either. 

What could have happened?   Why did the cashout not go through? 

To further this issue, I re-submitted a cashout request today so my account currently says "pending cashout July 20."

I still was able to click "cashout" again...so it seems the cashout did not go through the first time. It was pending for over a month and a half the first time I pressed cashout and then it appears to have reset itself as if i never pressed cashout. It's really strange.

That is odd - you need to send in a support ticket - it might take a few days to get an answer but they will help you.
