Survey issue

Why every time  I click a survey, instead of getting a survey, I get a "certified winner claim form"?

I'm not sure about the claim form but the three surveys that I just attempted took me to an error url page.   

Hopefully it's just a minor glitch because I have been enjoying very good success with the surveys on Fusion Cash over the last couple of weeks.

I did just complete an Opinion Central and am waiting for credit.
