If you do all their requirements, then nothing to worry that you don't get pay at all.

I've had  a survey payment revoked a time or two.  It happens.  I have no idea why mine were revoked, but I now try to have a cushion past the $25 minimum just in case that happens.  You will still get the rest of your money, but you will have to wait until the next month to get it.

Sometimes they claim you rushed through it or answered inconsistently. Sometimes that's not true. Don't let it get to you. Have some extra money before cashing out, and just move onto the next task.

Well... it can't be disapproved completely... there might be the chance that one or two of your payments get denied... which means that if you cash out  just and exactly 25.. and it goes under that amount, than they can't pay your for the month.
This is why when you go cash out they always tell you: make sure you have a bit over 25 (just in case).

So far I had a couple of surveys denied, but the final amount was always over 25, so I always got paid for the month.

It always states that when I cash out. No worries,  if you played by the rules you will be paid.

Once I had a survey payment revoked for whatever reason, I don't know.  My balance fell below the required amount and I had to wait and submit the next month.
