It is exactly the same.  I have seen it twice now.  The second time I just hit next without answering the questions and it went on with the survey.  If our answers are anonymous then I am not sure why they would need to go to such extreme lengths to confirm MY identity.  That just seems foolish.  Esp when you are just getting opinions on tooth paste.

This happened to me once but the answers were in multiple choice and it was on a different site. I closed out of it because I'm not comfortable giving that out

It's better to not make a little extra money, then to lose your monies, your,identity, basically you're life to scammers! Listen to your gut feelings. Never, never, give out your SSN!

That is very sketchy. I would advise not giving out your SSN in that manner.
