I am sure since it was your first visit in a while they want to update your history. It seems that just going to a new doctor requires answering a 3 page questionnaire.

Sorry to hear. Hope you are feeling better! But I think they probably want to make sure that they send the $5000 charge for just checking your pulse and see if you have a fever? The cause of emergency visit nowadays is killing me!

Sorry to hear that! I hope everything is okay. I was in the ER three times this past December and don't think they gave me any paperwork to fill out, but two out of those three times it was pretty serious and urgent, so they probably didn't want to bother! lol  Hope your health improves. :)

Thanks all! Feeling better - found out I have a concussion, but am getting treatment/further evaluation - saw my first bill and a bit nervous about the overall costs, but we'll see....and yeah, maybe because it was my first time they needed info, but it seemed like they kept asking me the same stuff over and over....

Going to the hospital is not easy but in the long run it's better to be safe than sorry.

I spent yesterday in an ICU waiting room. I was impressed with how well it was set up.

if you go to  hospital  or doctor it always takes a long time  both waiting for the actual doctor

What they do to us by making us fill out a magazine to see a doctor is so unnecessary. Don't get me wrong some questions are necessary, but the bulk is for propoganda purposes.

I got what you are saying. :)  Been visiting in the past and it very frustrating to spend time at the hospital.
