I'm wondering where did this militant group come from...and how did they become such a large menace? I thought we kept tabs on that part of the world....

They came from Iraq.  We, the USA, are not the world police.  Although it seems sometimes we think we are.  People in the Middle Eastern countries do not believe like we do in the US.  The Middle East is largely made up of muslims which is NOT a peaceful religion.  No religion that says you must believe like we do or we will kill you is a peaceful religion.  The evil in the world is only going to get worse until the end of time as we know it.
