Well the only way I know how to ask for a date is just ask. I was never one that got many yeses. But if you don't ask you will never know.

Open your mouth and just ask.  Hey do you want to go see a movie or mention a specific movie.  Do you want to go to ### and eat.  Or just do you want to go get something to eat.  It's not that big of a deal.  And even if they say no.  It's not that big of a deal.

I guess it would depend on how comfortable with a person you are?
Are you in a relationship already? Or do you want to be in one with someone?
If the second one I suppose just go up to them say you like them and if you'd want to go on a date with them sometime ( maybe have a day and time in mind could be good, while still being open to other times but I suppose that wouldn't be necessary.)
Really I think the best way either way is to just work up the courage and be blunt about it, might be uncomfortable to do, but I personally don't see any other way, but that's just me.

Maybe you could surprise them with roses and chocolate when asking, or something they like since not everybody likes receiving cliche' "romantic" things. Put a note in/on it asking to go on a date if you don't know how to say it out loud.

Honestly, this would probably make me mad.  I would rather a guy just say would you like to go to a movie or go get something to eat.  In the past it has always ticked me off for a guy to be like "oh I like you"  or   "I love you"  and I am like "you do not even know me."  I think it is best to just ask if they want to go some where.  And don't be offended if you want to go eat and they say I will meet you there.  I remember a few years ago a guy got really mad at me because I said I would meet him.  It's like Hey I don't know you.  You can't be too safe or too careful these days.

I think that if there is an activity you do together like work you could find a time to ask hey you want to get dinner sometime? Or else you will just have to do a cold call and just ask.
