Yes I completed the whole survey/task and wouldn't let me either. After a few attempts it finally took the code but then I never received credit. Maybe I should submit ticket also.

Can anyone please tell me, how / where do you get the codes?

The code is given to you at the very end of the survey, once you submit it. It will usually say something like ''Please enter this code: ...... in the box on the Task page in order to receive your reward''. You will never receive credit for any of these surveys if you don't enter the correct/official code.
