That's indeterminate since insufficient information was provided as to wht type of offer you're referring to, (which would separate any possibilities into either the 'Glitches Happen', various DQ reasons or, the closed-completion-page-before-offer-could-credit/rushed-through-survey/didn't-clear-cookies-before-starting-offer catagories for instance).

It is has only been recently that I haven't been credited for several, either.  Hoping it changes.  I will try cookies thing.
