Try clicking on the Special Offers tab, then 'offer status'. See if you find the Columbia House offer under COMPLETED OFFERS or if it is under OFFERS IN PROGRESS. If you find in under 'offers in progress' then click on the tiny question mark to the right of the offer in question. That will take you to a screen titled CONTACT CUSTOMER SERVICE. Fill out the form on that page with all the information you can provide and FC will respond within 24 hours. Be sure to click on "Send Request".

If you don't see the Columbia House offer as an OFFER IN PROGRESS, then I would file a Support Ticket with FC. There are many reasons why an offer may not credit, but FC will do their best to help if they can. Just remember that if FC doesn't get paid, then neither do you. Sometimes it's just out of their control. Good luck!
