Huh, I've never seen that completion screen for Surveyhead.  I just get a very basic "Thank You For Your Time," and I've never had a problem getting credit.  Something is definitely screwy with yours, but I don't know what would cause that.  Have you cleared your cookies?

I just had that EXACT same page show up for me 2 days ago. No credit and I always clear cookies before I do the surveys on here. I thought about copying/posting the screenshot, but figured it was a lost cause. It (Surveyhead) has done that to me 3 times now and never got credit when it happened that way vs the usual and simple "Congratulations" page.
