What is one of your most embarrassing moments that you can share?

One of the most embarrassing moments I had happened about 20 years ago.  I was working at an engineering firm and we had a project out for bid.  I was always getting prank calls from our guys (employees) when they called in from the field.  I answered a call one day when the caller inquired about the project being bid.  I asked his name and when he told me, my response to him was, "Sounds like a personal problem to me."  I was certain I was being pranked.  After a little bit of hesitation on his part, he said his full name.  His name was Richard Smailes (pronounced "smells") but went by the usual nickname for Richard (and not "Rich", either).  Would you have thought it was a prank?
