How do you get a cashout payment that was approved and then canceled credited back to your account?

It depends on why it was cancelled.  If there was an administrative reason, you need to resolve the omissions with Support in order to cash out.

If one of your credits was reversed and that reduced the balance of your cash out amount below the $25/15 requirement, then you just need to get your balance back up and request cash out again.  Check your account page to see what/if anything has changed.

If you do not see that anything has been reversed, you should contact Support to find out what caused them to reverse the cashout.

Normally, if a cashout is cancelled (by you or FC), the credits would be reinstated in your account (changed from pending cashout to approved).
